Managing multiple chronic conditions can feel like sailing in rough seas. If you are a Medicare patient dealing with two or more chronic diseases, Medicare has approved a program in which you can receive additional oversight and more frequent access to members of our care team. When you enroll in IMA’s SeniorPlus Program, a dedicated SeniorPlus nurse will contact you regularly to check on you, answer any questions you may have and offer help with other support you may need. The SeniorPlus nurse can also keep your doctor abreast of any significant changes to your health in between visits.
